Roles & Responsibilities

• Provide compassionate direct care and assistance with activities of daily living for geriatric residents
• Perform thorough patient checks and promptly report changes to the charge nurse or DON
• Accurately chart care and observations in MatrixCare to keep the team informed
• Support safe mobility and fall prevention efforts, using proper body mechanics
• Assist with meal support, hygiene, dressing, and toileting while respecting resident dignity
• Collaborate with nursing staff to follow care plans and participate in shift handoffs
• Participate in unit-specific orientation and ongoing training to maintain best practices
• Float to other units as needed, adapting quickly while maintaining quality care

Qualifications

• Current CNA certification in good standing
• BLS (Basic Life Support) certification required
• Minimum 2 years of experience in long term care, SNF, or post-acute settings preferred
• Comfortable working 12-hour night shifts and every-other-weekend schedule
• Experience with MatrixCare or similar EMR/charting systems is a plus
• Ability to follow facility policies, maintain resident confidentiality, and work as part of a team

Knowledge & Skills

• Geriatric care knowledge and sensitivity to age-related conditions
• Proficiency with MatrixCare charting system
• Strong communication skills with residents, families, and care teams
• Infection control and safety practices in a long term care environment
• Hands-on skills for ADLs, mobility assistance, and basic care procedures
• Ability to prioritize and escalate clinical concerns effectively
