Roles & Responsibilities

• Coordinate and execute comprehensive discharge planning for acute-care patients to achieve safe, timely transitions
• Perform utilization review to ensure appropriate level of care and effective resource use
• Document care and case management activity clearly and promptly in Cerner and related systems
• Collaborate with interdisciplinary teams, placement agencies, and family/support systems to secure appropriate post-acute placements
• Assist with SNF and lower-level placement navigation, including Medicare/Medi-Cal considerations
• Float across adult hospital areas as needed and support areas like women’s health when qualified
• Maintain professional standards on the unit (attendance, timely breaks/lunches, professional attire, minimal fragrance)

Qualifications

• Current California RN license (required)
• BLS certification (required)
• Minimum of 3 years hospital experience with acute-level care coordination experience
• Prior California Dignity/CommonSpirit acute care experience is mandatory
• Proficiency with Midas, InterCall, Cerner, Curaspan, and Microsoft Office
• Demonstrated experience in discharge planning and utilization review
• Two supervisory references required at submittal: one within 12 months and one within 3 years (charge accepted)
• Candidates may not have been directly employed by CommonSpirit/Dignity/CHI within the past year (PRN via outside firms may be considered)

Education

• Active RN license in California
• BLS certification required
• BSN not required

Knowledge & Skills

• Strong knowledge of acute care case management and care coordination
• Familiarity with Medicare/Medi-Cal guidelines related to SNF and lower-level placements
• Experience working with placement agencies and arranging socially-impacted discharges
• Solid charting skills and comfort with electronic health records (Cerner)
• Excellent communication, adaptability, critical thinking, and attendance reliability
• Ability to work efficiently in a high-needs, underserved community setting
