Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long-term care setting
• Administer medications and treatments accurately and safely following orders
• Assess resident condition and document changes in health status clearly
• Collaborate with the care team to develop and update individualized care plans
• Educate residents and families about care, medications, and health goals
• Support safe infection control and maintain a clean, therapeutic environment
• Respond to resident needs and emergencies with professionalism and calm

Qualifications

• Current Licensed Practical Nurse (LPN) license — Rhode Island license required or eligible
• Current BLS/CPR certification (or willing to obtain prior to start)
• Up-to-date immunizations and TB screening as required by facility policy
• Ability to pass background check and drug screening per facility guidelines
• Previous experience in long-term care or skilled nursing preferred

Education

• Graduate of an approved Practical Nursing program (LPN/LVN)

Knowledge & Skills

• Strong medication administration and documentation skills
• Proficient in basic assessments: vitals, wound checks, and ADL support
• Comfortable working within an interdisciplinary team
• Clear, compassionate communication with residents and families
• Good time management and ability to prioritize resident needs
• Familiarity with electronic health record charting and care plans
