Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a long-term care setting
- Administer medications and treatments accurately and safely following orders
- Assess resident condition and document changes in health status clearly
- Collaborate with the care team to develop and update individualized care plans
- Educate residents and families about care, medications, and health goals
- Support safe infection control and maintain a clean, therapeutic environment
- Respond to resident needs and emergencies with professionalism and calm
Qualifications
- Current Licensed Practical Nurse (LPN) license — Rhode Island license required or eligible
- Current BLS/CPR certification (or willing to obtain prior to start)
- Up-to-date immunizations and TB screening as required by facility policy
- Ability to pass background check and drug screening per facility guidelines
- Previous experience in long-term care or skilled nursing preferred
Education
- Graduate of an approved Practical Nursing program (LPN/LVN)
Knowledge & Skills
- Strong medication administration and documentation skills
- Proficient in basic assessments: vitals, wound checks, and ADL support
- Comfortable working within an interdisciplinary team
- Clear, compassionate communication with residents and families
- Good time management and ability to prioritize resident needs
- Familiarity with electronic health record charting and care plans
