Roles & Responsibilities

Provide compassionate bedside care and daily living assistance to residents
Administer medications and treatments accurately and document accordingly
Perform routine assessments and report changes in patient condition promptly
Collaborate with RNs, therapists, and interdisciplinary team members to deliver coordinated care
Maintain accurate, timely charting in the patient record
Adhere to infection control and safety protocols to protect patients and staff
Support residents with activities of daily living while promoting dignity and independence

Qualifications

Valid Licensed Practical Nurse (LPN) license for Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain)
Comfortable with medication administration, wound care basics, and resident assessments
Strong communication skills and ability to work as part of a multidisciplinary team
Reliable, compassionate, and patient-focused approach to care

Education

Practical Nursing diploma or vocational certificate
Licensed Practical Nurse (LPN) licensure required

Knowledge & Skills

Skilled in medication administration and basic clinical procedures
Proficient in taking and recording vital signs and monitoring changes
Familiarity with resident care documentation and basic charting practices
Knowledge of infection control standards and patient safety practices
Good interpersonal skills and the ability to educate residents and families
