Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in line with care plans
• Administer medications and treatments accurately and safely
• Monitor and document vital signs, patient status changes, and clinical observations
• Collaborate with nurses, physicians, and interdisciplinary team members to deliver coordinated care
• Educate residents and families about care plans, medications, and follow-up instructions
• Maintain thorough, timely documentation in the facility EHR and adhere to policies
• Identify and escalate clinical concerns promptly to ensure resident safety
• Practice infection control, safety protocols, and maintain a clean care environment

Qualifications

• Active Licensed Practical Nurse (LPN) license — ability to practice in Rhode Island required
• Current BLS/CPR certification or willingness to obtain upon hire
• Completion of an accredited practical nursing program (diploma or certificate)
• Comfortable working in a long-term care or skilled nursing environment
• Strong communication skills, teamwork mindset, and professional attitude
• Ability to pass background check and any required health screenings

Education

• Graduate of an accredited Practical Nursing program (diploma or certificate)

Knowledge & Skills

• Medication administration and safe documentation practices
• Resident assessment and basic clinical evaluation skills
• Wound care fundamentals and monitoring for changes
• Accurate charting and familiarity with electronic health record systems
• Infection prevention, safe patient handling, and fall risk mitigation
• Clear patient and family education, plus compassionate bedside manner
• Ability to prioritize tasks in a fast-paced care setting
