Roles & Responsibilities

• Provide compassionate, resident-centered speech-language therapy services in a skilled nursing setting
• Assess, evaluate and create individualized treatment plans that improve communication and swallowing
• Document therapy sessions and progress clearly and timely to meet facility and CRG standards
• Collaborate with nurses, therapists and care teams to deliver coordinated rehabilitation
• Meet productivity expectations and contribute to a high-performing therapy program
• Represent the facility professionally (black scrubs dress code) and maintain on-site presence as required

Qualifications

• 1–3 years of experience in speech-language pathology (as noted in the job requirements)
• Experience working in a Skilled Nursing Facility (SNF) or similar long-term care environment
• Ability to work on-site and be present in the building to receive guaranteed hours
• Comfort following Continuum Rehabilitation Group (CRG) policies & procedures and meeting productivity targets
• Willingness to wear black scrubs per facility dress code and maintain a professional appearance

Knowledge & Skills

• Skilled in SLP clinical assessment and treatment for adult/geriatric populations
• Familiarity with SNF therapy workflows, documentation and productivity requirements
• Strong interdisciplinary communication and collaboration skills
• Clinical problem-solving and resident-centered care planning
• Reliable time management and on-site professionalism
