Roles & Responsibilities
- Provide compassionate, high-quality nursing care to residents and patients
- Administer medications and treatments according to orders and facility policy
- Document clinical assessments, interventions, and resident responses accurately
- Monitor resident conditions and promptly report changes to the care team
- Collaborate with nurses, therapists, and providers to implement individualized care plans
- Assist residents with activities of daily living while promoting independence and dignity
Qualifications
- Current Licensed Practical Nurse (LPN) license in Pennsylvania
- Completion of an accredited practical nursing program
- Basic Life Support (BLS) certification preferred or obtainable upon hire
- Comfortable with medication administration, charting, and basic clinical procedures
- Strong communication, teamwork, and resident-centered care approach
- Previous experience in long-term care or skilled nursing is a plus
Education
- Graduate of an accredited Practical Nursing program
- High school diploma or GED
Knowledge & Skills
- Medication administration and accurate documentation
- Vital signs assessment and basic clinical monitoring
- Familiarity with infection control and safety protocols
- Ability to work with electronic health records and charting systems
- Good judgment, time management, and prioritization skills
- Strong interpersonal skills for working with residents and families
