Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments safely and accurately
- Monitor and document patient conditions, vitals, and progress notes
- Collaborate with nurses, physicians, and care teams to deliver coordinated care
- Assist with ADLs (activities of daily living) while preserving patient dignity
- Observe infection control protocols and maintain a safe care environment
- Communicate clearly with patients and families about care plans and changes
- Participate in shift handoffs and ensure continuity of care
Qualifications
- Active LPN license (practical nurse) — Pennsylvania license preferred
- Current basic life support (BLS) / CPR certification or willingness to obtain
- Previous LPN experience in long-term care, skilled nursing, or similar setting preferred
- Strong clinical assessment, medication administration, and documentation skills
- Ability to work compassionately with residents and collaborate with a multidisciplinary team
- Clear background check and ability to meet facility onboarding requirements
Education
- Diploma or certificate from an accredited LPN program
- High school diploma or equivalent
Knowledge & Skills
- Patient assessment and vital signs monitoring
- Medication administration and accurate charting
- Familiarity with infection control and safety procedures
- Basic wound care and dressing change knowledge
- Comfortable using electronic health records and documentation systems
- Strong communication, organizational, and time-management skills
