Contract Type: Per-Diem

Shift Type: 8-hour shifts; exact start/end times and scheduled days to be confirmed

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care consistent with LPN scope of practice
Administer medications and document accurately in the medical record
Monitor patient status, report changes to the RN or provider, and escalate concerns promptly
Assist with activities of daily living and ensure patient comfort and safety
Collaborate with interdisciplinary team members to support individualized care plans
Maintain clear, timely charting and uphold confidentiality and compliance standards
Support admissions, discharges, and patient transfers as needed to ensure smooth operations

Qualifications

Current, active LPN license in Pennsylvania (or compact/reciprocal as applicable)
CPR/BLS certification required
Previous LPN experience in a long-term care or similar clinical setting preferred
Ability to pass background check and meet facility health/vaccination requirements
Strong communication skills and the ability to work effectively as part of a team

Education

LPN diploma or certificate from an accredited program
High school diploma or GED

Knowledge & Skills

Skilled in medication administration and basic wound care
Comfortable with patient assessments and documenting changes
Familiarity with electronic medical records (EMR) or charting systems
Good clinical judgment, time management, and prioritization skills
Demonstrated empathy and strong patient-centered care focus
