Roles & Responsibilities
- Provide compassionate bedside care and promote resident dignity and comfort
- Administer medications and prescribed treatments according to care plans
- Monitor and record vital signs and report clinical changes promptly
- Maintain accurate, timely documentation in the medical record
- Collaborate with nurses, therapists, and support staff to deliver coordinated care
- Assist with admissions, discharges, and safe transfers as needed
Qualifications
- Current LPN license in Pennsylvania (or eligible for state licensure)
- Basic Life Support (BLS) certification preferred or required depending on facility policy
- Strong clinical assessment and medication administration skills
- Excellent communication skills and a team-oriented mindset
- Ability to stand for shifts, perform hands-on resident care, and document accurately
Education
- Graduate of an accredited practical nursing program (LPN certificate or diploma)
Knowledge & Skills
- Medication administration and accurate charting
- Fundamental nursing assessments and vital sign monitoring
- Infection control and safe patient handling practices
- Clear, compassionate communication with residents and families
- Ability to work effectively within an interdisciplinary care team
