Roles & Responsibilities
- Provide compassionate bedside care and monitor resident condition closely
- Administer medications and treatments accurately and safely
- Document assessments, interventions, and resident responses in the chart
- Collaborate with RN team members and interdisciplinary staff to deliver great care
- Assist with activities of daily living and support resident comfort and dignity
- Report changes in condition promptly and participate in care planning
- Promote a safe, clean environment and follow infection control practices
Qualifications
- Active Licensed Practical Nurse (LPN) license valid in Pennsylvania
- Current Basic Life Support (BLS) certification (or willingness to obtain)
- Comfort with medication administration, charting, and resident assessments
- Strong communication skills and a team-focused attitude
- Ability to pass background and health screening per facility policy
Education
- Practical Nursing diploma or certificate (LPN program graduate)
Knowledge & Skills
- Skilled in clinical assessments and basic nursing procedures
- Medication administration and documentation best practices
- Familiarity with long-term care or skilled nursing workflows
- Ability to use electronic medical records for accurate charting
- Knowledge of infection control, resident safety, and care planning
