Roles & Responsibilities
- Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
- Administer medications and treatments safely and accurately following facility policies
- Monitor patient status and report changes promptly to RN/MD to ensure quality outcomes
- Document assessments, interventions, and patient responses clearly and timely
- Assist with activities of daily living while promoting dignity and independence
- Collaborate with interdisciplinary team members to support individualized care plans
- Participate in unit initiatives to improve patient experience and operational efficiency
Qualifications
- Current and active LPN license in Pennsylvania (or compact/state-appropriate) required
- CPR/BLS certification required; additional certifications a plus
- Previous LPN clinical experience preferred; new grads with strong clinical training encouraged to apply
- Ability to pass background checks and meet facility onboarding requirements
- Strong communication skills and a patient-first mindset
Education
- Graduate of an accredited Practical Nursing program (LPN diploma or equivalent)
Knowledge & Skills
- Solid medication administration and documentation skills
- Basic assessment and vital signs monitoring
- Familiarity with electronic medical records and charting best practices
- Ability to triage concerns and escalate appropriately
- Teamwork, time management, and prioritization in a fast-paced setting
