Roles & Responsibilities

• Provide in-home physical therapy services to maximize patient independence and safety
• Perform thorough patient assessments and develop individualized care plans
• Deliver skilled interventions and progress patients toward functional goals
• Collaborate with a multidisciplinary team to coordinate holistic patient care
• Document visits and clinical progress accurately and timely
• Educate patients and caregivers on home strategies to maintain safety and mobility
• Manage a travel schedule to visit patient homes reliably and safely

Qualifications

• VT Physical Therapy license (required)
• Valid driver’s license and reliable vehicle (required)
• Experience in a home care setting preferred
• Comfortable working independently in patient homes while staying connected to the team
• Experience with hospice or home health populations is a plus

Knowledge & Skills

• Skilled in patient assessment and individualized care planning
• Experience with home health and in-home therapy best practices
• Knowledge of mobility, fall prevention, and safety adaptations in the home
• Strong documentation and clinical reasoning skills
• Excellent communication and patient/caregiver education abilities
• Ability to manage time and a caseload of in-home visits
