Roles & Responsibilities
- Provide compassionate direct patient care to residents at Bear Canyon Rhb Ctr
- Administer medications and treatments safely and accurately
- Perform thorough patient assessments and promptly report changes
- Document care clearly and timely in the electronic medical record
- Collaborate with nurses, therapists, and providers to execute care plans
- Assist residents with activities of daily living and promote independence
- Adhere to infection control standards and facility policies
- Support transitions of care and communicate effectively with families
Qualifications
- Current and active Licensed Practical Nurse (LPN) license in New Mexico required
- Basic Life Support (BLS) certification required
- Prior experience in long-term care or rehabilitation settings preferred
- Comfort working overnight shifts and flexible availability
- Ability to pass background check and drug screen as required by facility
Education
- Graduation from an accredited practical nursing program (LPN diploma or certificate)
Knowledge & Skills
- Strong medication administration and documentation skills
- Patient assessment and vital signs monitoring
- Knowledge of long-term care clinical practices and regulatory standards
- Comfortable using electronic medical records and charting
- Effective communication and teamwork with interdisciplinary staff
- Time management and prioritization during busy night shifts
