Roles & Responsibilities

• Provide skilled rehabilitation interventions under the direction of the supervising physical therapist
• Deliver compassionate, person-centered care to SNF residents to improve mobility and function
• Assist with treatment plans, progress notes, and documentation to ensure quality continuity of care
• Coach residents and families on safe mobility, fall prevention, and home exercise programs
• Collaborate with nurses, therapists, and interdisciplinary team members to meet patient goals

Qualifications

• Current Physical Therapist Assistant (PTA) license or eligible for licensure in Michigan
• Graduate of an accredited PTA program
• CPR/BLS certification required or obtain upon hire
• Experience in skilled nursing facility (SNF) or geriatric populations preferred
• Strong communication skills and a team-first attitude

Education

• Associate degree from an accredited Physical Therapist Assistant program

Knowledge & Skills

• Knowledge of therapeutic modalities, gait training, transfers, and ADL assistance
• Skilled in documentation and progress note completion
• Ability to follow care plans and modify treatment under PT supervision
• Familiarity with patient safety, infection control, and long-term care regulations
• Comfortable working collaboratively in an interdisciplinary setting
