Roles & Responsibilities
- Provide compassionate, patient-centered nursing care in accordance with plan of care
- Administer medications and treatments safely and accurately
- Perform thorough patient assessments and document findings clearly
- Collaborate with the interdisciplinary team to coordinate care
- Monitor patient status and escalate changes promptly to RN/physician
- Support patients and families with education and emotional reassurance
- Maintain accurate and timely clinical documentation in the facility charting system
Qualifications
- Current LPN license (eligible to practice in New Hampshire preferred)
- CPR/BLS certification required or obtained upon hire
- Immunizations and TB screening up to facility policy
- Comfortable with medication administration, wound care, and basic IV-related tasks as required by facility
- Demonstrated ability to communicate clearly and work well in a team
- Willingness to pass background check and meet facility onboarding requirements
Education
- Graduate of an accredited Practical Nursing program
- Current LPN licensure
Knowledge & Skills
- Strong clinical assessment and vital signs monitoring skills
- Proficient in medication administration and documentation
- Familiarity with electronic medical records and charting best practices
- Ability to prioritize tasks in a fast-paced healthcare setting
- Patient education and compassionate bedside manner
- Basic wound care and infection control practices
