Roles & Responsibilities
- Conduct screening, clinical assessments, diagnoses, treatment planning and brief therapy across the lifespan
- Lead and facilitate discharge planning and transitions to ALF or other transitional placements
- Coordinate care with hospital teams and community partners to ensure appropriate services and smooth transitions
- Provide client advocacy, skill-building, and assistance with complex issues such as Power of Attorney
- Document screenings, assessments, treatment plans, progress notes and required reports in a timely and compliant manner
- Support patients with chemical dependency and mental health needs and make appropriate referrals
- Participate in multidisciplinary team meetings and co-facilitate patient care conferences
- Complete assigned billing duties and provider enrollment documentation as requested
Qualifications
- Master's degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution
- Licensed (or in process of licensure) as LCSW, LPC, or LMFT
- Valid State of Alaska driver’s license or ability to obtain within 6 months of hire
- Must be 21 years of age
- Achieve Clinical Competency within 3 months of hire and maintain competency every three years
- Ability to perform case management, crisis intervention, clinical documentation and patient-centered advocacy
- Willingness to participate in SEARHC quality management trainings and comply with accreditation and state/federal regulations
- Required certification: Clinical Competency Assessment - SEARHC
Education
- Master's Degree in Social Work, Psychology, Counseling or related discipline
Knowledge & Skills
- Discharge planning best practices and care coordination
- Interviewing techniques and crisis intervention methods
- Clinical documentation standards for hospital billing and accreditation
- Ethical and legal management practices and patient rights
- Ability to observe, evaluate and document psychiatric and substance use behaviors
- Experience working with patients across the lifespan (birth through elder)
- Familiarity with tribal health and cultural wellness approaches
- Comfort working in multidisciplinary teams and community partnerships
