Roles & Responsibilities

• Evaluate patients' mobility, strength, balance and functional needs and create individualized treatment plans
• Provide hands-on and therapeutic interventions to improve patient outcomes in the SNF setting
• Document patient progress clearly and timely in the medical record
• Collaborate with nurses, OT, SLP and the interdisciplinary team to coordinate care
• Educate patients and families on home exercise programs and safety strategies
• Monitor clinical outcomes and adjust treatment plans to meet goals
• Support safe transfers, discharge planning, and continuity of care

Qualifications

• Doctor of Physical Therapy (DPT) or Physical Therapist (PT) degree — DPT preferred
• Licensed or eligible for licensure as a Physical Therapist in Oklahoma
• Comfortable working in a Skilled Nursing Facility (SNF) environment
• Open to all experience levels — new grads welcome
• Strong communication and teamwork skills; dependable and patient-focused

Education

• Doctor of Physical Therapy (DPT) preferred; PT degree acceptable
• New graduates encouraged to apply

Knowledge & Skills

• Skilled in patient assessment, gait training, therapeutic exercise and balance programs
• Experience with SNF documentation and rehab-focused care
• Ability to create functional, person-centered treatment plans
• Good clinical reasoning, time management and multitasking in a busy setting
• Familiarity with common rehab equipment and safe transfer techniques
