Roles & Responsibilities

• Evaluate patients' physical function and develop individualized treatment plans
• Implement therapeutic interventions including exercise, manual therapy, and gait training
• Document progress clearly and timely in the patient record
• Collaborate with interdisciplinary teams to coordinate patient care and achieve goals
• Educate patients and families on home programs and strategies to maximize recovery
• Monitor outcomes and modify plans of care based on patient response
• Participate in team meetings and contribute to a positive, patient-centered environment

Qualifications

• Current license to practice Physical Therapy in New Hampshire, or ability to obtain one prior to start
• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy accepted where applicable
• Basic Life Support (BLS) certification required or obtained upon hire
• Clinical experience in inpatient or outpatient rehab settings preferred
• Strong patient communication and documentation skills
• Reliable, professional, and able to work effectively within a multidisciplinary team

Education

• Doctor of Physical Therapy (DPT) preferred
• Master’s or Bachelor’s degree in Physical Therapy where applicable

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy, and functional mobility training
• Experience with patient assessment, care planning, and progress tracking
• Comfortable using electronic medical records for documentation
• Strong clinical reasoning and problem-solving abilities
• Excellent interpersonal skills and patient education techniques
• Ability to prioritize caseload and manage time effectively
