Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents and patients during the assigned shift
• Administer medications and treatments accurately and document per facility policy
• Perform routine assessments, monitor vital signs, and report clinical changes promptly
• Collaborate with RNs, providers and interdisciplinary team members to support patient-centered plans
• Maintain clear, timely documentation in the medical record and communicate effectively at handoff
• Support patient and family education with empathy and clear explanations
• Follow infection control protocols and facility policies to ensure a safe care environment

Qualifications

• Active LPN license in Maryland (required)
• Previous experience in long-term care, skilled nursing, or similar setting is a plus
• Current Basic Life Support (BLS) certification or willingness to obtain
• Strong communication and teamwork skills; dependable and professional
• Ability to pass required background checks and meet facility onboarding requirements

Education

• Graduate of an accredited LPN / Practical Nursing program
• High school diploma or equivalent required

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and monitoring skills
• Skilled at wound care, ADL assistance, and resident-centered care
• Familiarity with electronic medical records and charting
• Knowledge of infection prevention and safety protocols
• Ability to prioritize tasks in a busy clinical environment and escalate concerns appropriately
