Roles & Responsibilities

• Assist licensed physical therapists in delivering skilled rehab interventions and therapeutic exercises
• Implement treatment plans, document patient progress, and communicate changes to the therapy team
• Provide safe mobility and transfer assistance while teaching patients functional techniques
• Use clinical judgment to adapt treatments under the PT’s direction for optimal patient outcomes
• Support discharge planning and provide clear patient and caregiver education
• Collaborate with nurses, physicians and interdisciplinary staff to ensure coordinated care

Qualifications

• Active California Physical Therapist Assistant license or eligibility to obtain one
• Graduation from an accredited PTA program
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Comfortable floating between Alameda and Park Bridge as needed
• Strong communication skills and a patient-centered approach
• Prior experience in acute care, inpatient rehab or skilled nursing preferred

Education

• Associate degree from an accredited Physical Therapist Assistant program (required)

Knowledge & Skills

• Skilled in therapeutic exercises, gait training and mobility assistance
• Competent with patient documentation and electronic medical records
• Knowledge of fall prevention, safe transfer techniques and assistive devices
• Ability to follow and implement PT treatment plans accurately
• Strong interpersonal skills for teaming with clinicians and educating patients
