Roles & Responsibilities

Provide high-quality, evidence-based physical therapy services to short-term rehab and long-term care residents
Evaluate functional status and develop individualized treatment plans that promote recovery and independence
Document progress clearly and timely in the resident chart and communicate with the interdisciplinary team
Collaborate with nurses, therapists, and physicians to coordinate care and optimize outcomes
Educate residents and families on home programs, fall prevention, and safe mobility
Contribute to a positive facility culture by mentoring support staff and participating in quality initiatives

Qualifications

Active Rhode Island Physical Therapist license required
Doctor of Physical Therapy (DPT) or equivalent degree preferred
Current CPR/BLS certification
Experience in a skilled nursing or post-acute rehab setting preferred
Ability to work the assigned day shift schedule and required weekend rotation
Strong documentation skills and comfort with electronic medical records

Education

Doctor of Physical Therapy (DPT) or equivalent degree

Knowledge & Skills

Skilled knowledge of mobility, balance, gait training, and therapeutic exercise
Familiarity with Medicare/Medicaid and SNF documentation requirements
Ability to perform safe patient handling and fall-risk assessments
Excellent communication skills and a collaborative approach to care
Time management and caseload prioritization in a fast-paced setting
