Roles & Responsibilities
- Provide compassionate direct patient care and support daily living activities
- Administer medications and follow care plans safely and accurately
- Monitor and document patient vitals and clinical changes in the chart
- Collaborate with RNs, physicians, and the care team to deliver excellent outcomes
- Educate patients and families on care plans, medications, and discharge instructions
- Maintain a clean, safe environment and adhere to infection control policies
- Advocate for patients' needs and contribute to a positive patient experience
Qualifications
- Active LPN license in Pennsylvania required
- Graduate of an accredited LPN program
- Current CPR/BLS certification required or obtained upon hire
- Comfortable administering medications, documenting care, and performing assessments
- Strong communication skills and ability to work well on a collaborative team
- Previous LPN experience in long-term care, skilled nursing, or acute care preferred
Education
- Graduate of an accredited Licensed Practical Nurse program
- High school diploma or GED required
Knowledge & Skills
- Skilled in medication administration and basic wound care
- Able to perform patient assessments and recognize changes in condition
- Competent with clinical documentation and charting
- Knowledgeable about infection control standards and safety protocols
- Team-oriented with strong interpersonal and time management skills
