Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
Administer medications and treatments accurately and document all care
Collaborate with the interdisciplinary team to develop and follow individualized care plans
Monitor resident condition and promptly report changes to RN/supervisor
Perform routine assessments, wound care, and assist with activities of daily living
Communicate clearly with residents and families to ensure comfort and understanding
Follow facility policies, safety protocols, and infection control standards
Contribute to a positive workplace culture by mentoring peers and sharing best practices

Qualifications

Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
Current Basic Life Support (BLS) / CPR certification (or willingness to obtain)
Prior experience in skilled nursing or long-term care preferred
Ability to pass background check and meet facility health/immunization requirements
Strong communication, organization, and resident-centered care skills

Education

Practical Nursing diploma or certificate (LPN program)
High school diploma or GED

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and infection prevention techniques
Familiarity with electronic health records or paper charting
Ability to perform resident assessments and recognize changes in condition
Teamwork, time management, and patient advocacy
