Contract Type: Per-Diem

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments safely and accurately
Monitor and document patient conditions, vitals, and progress notes
Collaborate with nurses, physicians, and care teams to deliver coordinated care
Assist with ADLs (activities of daily living) while preserving patient dignity
Observe infection control protocols and maintain a safe care environment
Communicate clearly with patients and families about care plans and changes
Participate in shift handoffs and ensure continuity of care

Qualifications

Active LPN license (practical nurse) — Pennsylvania license preferred
Current basic life support (BLS) / CPR certification or willingness to obtain
Previous LPN experience in long-term care, skilled nursing, or similar setting preferred
Strong clinical assessment, medication administration, and documentation skills
Ability to work compassionately with residents and collaborate with a multidisciplinary team
Clear background check and ability to meet facility onboarding requirements

Education

Diploma or certificate from an accredited LPN program
High school diploma or equivalent

Knowledge & Skills

Patient assessment and vital signs monitoring
Medication administration and accurate charting
Familiarity with infection control and safety procedures
Basic wound care and dressing change knowledge
Comfortable using electronic health records and documentation systems
Strong communication, organizational, and time-management skills
