Roles & Responsibilities

Provide compassionate, direct patient care consistent with LPN scope of practice
Administer medications and treatments safely and accurately
Monitor patients’ conditions and report changes to the care team
Document care and patient responses clearly in the medical record
Collaborate with RNs, providers and support staff to deliver coordinated care
Support a positive patient experience through clear communication and empathy

Qualifications

Valid LPN license (Licensed Practical Nurse) — required
Active authorization to practice in Pennsylvania (state licensure)
Current basic life support (BLS/CPR) certification preferred or required per facility policy
Strong clinical assessment and medication administration skills
Reliable, professional, and patient-focused with good communication skills

Education

Practical Nursing diploma or equivalent (LPN program completion)
High school diploma or GED required

Knowledge & Skills

Solid knowledge of LPN scope of practice and nursing fundamentals
Medication administration and documentation best practices
Patient assessment, vital signs monitoring, and basic wound care
Electronic medical record (EMR) documentation familiarity
Ability to multitask, prioritize, and work collaboratively in a fast-paced setting
