Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, patient-centered care to residents and patients
Administer medications and treatments accurately and document thoroughly
Perform routine assessments, monitor vital signs, and report changes promptly
Collaborate with nurses, aides, and other care team members to coordinate care
Maintain accurate, timely charting in the facility record system
Support infection control, safety protocols, and a clean care environment
Educate patients and families with clear, empathetic communication
Contribute to quality improvement initiatives and daily unit operations

Qualifications

Current LPN license in Pennsylvania (required)
Completion of an accredited LPN/practical nursing program
Current CPR/BLS certification or willingness to obtain
Prior LPN experience in long-term care, rehab, or similar setting preferred
Clear background check and ability to meet facility onboarding requirements
Strong communication, critical thinking, and teamwork skills

Education

Graduate of an accredited LPN/practical nursing program; active PA LPN license required

Knowledge & Skills

Medication administration and safe documentation practices
Basic wound care and dressing management
Patient assessment and recognizing acute changes
Familiarity with electronic medical records or ability to learn quickly
Infection control and standard precautions
Time management and prioritization during busy shifts
Collaborative, respectful teamwork and patient advocacy
