Roles & Responsibilities

• Provide compassionate, resident-centered nursing care to promote comfort and dignity
• Administer medications and treatments accurately and safely
• Perform routine assessments and document changes in condition promptly
• Collaborate with nurses, therapists, and interdisciplinary team members to implement care plans
• Educate residents and families about care, medications, and safety practices
• Assist with activities of daily living while encouraging resident independence
• Adhere to infection control, safety protocols, and facility policies to protect residents and staff

Qualifications

• Current Licensed Practical Nurse (LPN) credential in good standing
• Completion of an accredited LPN training program
• Previous experience in long-term care or rehab is a plus
• Strong interpersonal skills and the ability to work well on a multidisciplinary team
• Current BLS/CPR certification preferred or able to obtain

Education

• Graduate of an accredited LPN/Vocational Nursing program; high school diploma or GED required

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and charting skills
• Familiarity with long-term care/rehab workflows and resident care plans
• Comfortable using electronic health records and standard nursing documentation
• Good time management, prioritization, and communication skills
