Roles & Responsibilities

Conduct thorough psychosocial and clinical assessments for individuals and families to identify needs and strengths
Develop individualized treatment plans rooted in evidence-based therapeutic interventions
Provide individual and family therapy across the lifespan, addressing depression, anxiety, trauma, substance use, and adjustment issues
Collaborate closely with primary care providers, nurses, and behavioral health colleagues to coordinate whole-person care
Document clinical encounters and treatment progress clearly and timely to support continuity of care
Support care coordination and referral navigation for patients in rural and medically underserved settings

Qualifications

Master's degree in Social Work from an accredited program (MSW)
Active LISW license in Iowa
Minimum of 2 years of clinical social work experience preferred
Experience or strong interest in serving rural or medically underserved populations is a plus
Strong interpersonal, organizational, and documentation skills

Education

Master's degree in Social Work (MSW) from an accredited program
