Location: Smethport, PA

Start Date: 05-11-2026

Contract Type: Per-Diem

Shift Type: 8-hour shifts; days vary and specific start/end times to be confirmed (Total hours per shift: 8)

Hours per Week: 40

Candidate Pay Rate: $20 per hour

Roles & Responsibilities

Provide compassionate hands-on personal care and assistance with activities of daily living
Monitor residents' conditions and promptly report changes to nursing staff
Assist with safe transfers, mobility, and ambulation to promote independence
Support mealtime and hydration needs while encouraging dignity and choice
Maintain clean, safe resident environments and follow infection control protocols
Document care accurately and communicate effectively with the care team
Build trusting relationships with residents and families through respectful, person-centered care

Qualifications

Valid CNA certification in good standing (PA preferred)
Current BLS/CPR certification or willingness to obtain
Demonstrated compassion, reliability, and strong communication skills
Ability to perform basic patient care tasks and follow care plans
Willingness to pass required background checks and health screenings

Education

High school diploma or equivalent
Completion of a state-approved CNA program

Knowledge & Skills

Knowledge of activities of daily living (ADLs) and basic patient care
Understanding of infection control and safety best practices
Ability to observe, report, and document changes in resident condition
Strong interpersonal skills and the ability to work as part of a team
Time management and basic charting/recordkeeping skills
