Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care with attention to dignity and comfort
Administer medications and treatments safely and accurately per facility policy
Monitor and record vital signs and patient conditions, escalating concerns promptly
Document assessments and care provided clearly in the patient record
Collaborate with nurses, therapists and care team members to deliver coordinated care
Assist patients with activities of daily living and promote mobility and independence

Qualifications

Current, active LPN (Licensed Practical Nurse) license — ability to practice in Pennsylvania is a big plus
BLS/CPR certification or willingness to obtain upon hire
Reliable clinical judgment and comfort working in a fast-paced care environment
Strong communication skills and a team-focused attitude
Previous LPN experience preferred; new grads who meet licensure and certification requirements are welcome to apply

Education

Graduate of an accredited LPN/LVN program or equivalent

Knowledge & Skills

Medication administration and documentation best practices
Basic patient assessment and vital sign monitoring
Wound care fundamentals and infection control awareness
Electronic medical record (EMR) documentation skills
Time management, prioritization and clear clinical communication
