Roles & Responsibilities

• Perform thorough evaluations to assess patients' physical abilities and design individualized treatment plans
• Deliver targeted physical therapy interventions to improve strength, mobility, and functional independence
• Educate patients and families on therapeutic exercises, safe movement techniques, and home modifications
• Collaborate with occupational therapists, nurses, physicians, and other team members to coordinate care
• Monitor patient progress, adjust treatment plans as needed, and document outcomes
• Maintain accurate, timely documentation in accordance with facility policies and regulatory requirements
• Promote patient-centered care that enhances quality of life and supports safe discharge planning

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in California
• Experience in a skilled nursing facility or similar post-acute setting preferred
• Strong knowledge of therapeutic techniques, modalities, and adaptive equipment
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work independently while contributing positively to an interdisciplinary team
• Compassionate, patient-focused approach to care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy
