Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to maximize functional outcomes
• Develop and implement individualized treatment plans focused on meaningful patient goals
• Collaborate closely with an interdisciplinary team to coordinate seamless resident care
• Document evaluations, progress notes, and discharge plans accurately and in a timely manner
• Participate in care conferences and advocate for residents’ therapy needs
• Contribute to quality improvement initiatives to help raise clinical standards and resident satisfaction

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical skills with the ability to assess and treat post-acute and skilled nursing residents
• Excellent communication skills for working with residents, families, and interdisciplinary teams
• Skilled nursing or post-acute experience preferred
• Dependable, flexible, and team-oriented mindset
