Roles & Responsibilities
- Establish collaborative practice programs with oncologists to optimize chemotherapy and supportive care
- Provide targeted staff education for pharmacy, nursing, and medical teams to improve oncology care
- Serve as the oncology subject-matter expert, answering drug information questions and ensuring EHR care plans align with national guidelines
- Round with oncology fellows, nurse practitioners, and physicians to recommend medication adjustments and monitoring
- Deliver patient-specific medication education for oral oncolytics and IV chemotherapy using teach-back methods
- Review and update oncology care plans in the Electronic Health Record to ensure appropriateness and safety
- Identify and resolve medication-related problems using clinical data and lab results
- Precept pharmacy residents and students, helping to train the next generation of oncology pharmacists
- Participate in Oncology Service Line meetings to improve safety, efficacy, and quality of cancer care
- Promote cost-effective drug therapy and lead cost-avoidance initiatives within the Oncology Service Line
Qualifications
- Doctor of Pharmacy (PharmD) required
- Licensed Pharmacist in good standing
- Board Certified Oncology Pharmacist (BCOP) required
- Completion of PGY-1 residency plus clinical pharmacy training OR completion of PGY-2 oncology residency (or equivalent experience)
- Minimum of three years of hospital experience with three years of oncology practice/training required
- Preferred: PGY-2 ASHP-accredited residency and BPS oncology certification; candidates with 5+ years oncology experience and BCOP encouraged to apply
- Strong communication and interdisciplinary collaboration skills
Education
- Doctor of Pharmacy (PharmD) required
- PGY-1 residency (or equivalent clinical pharmacy experience) required; PGY-2 Oncology residency preferred
Knowledge & Skills
- Chemotherapy management and supportive care expertise
- Clinical guideline review and application for oncology practice
- Electronic Health Record (EHR) review and care-plan optimization
- Patient-specific medication education and teach-back techniques
- Drug information and medication safety principles
- Collaborative practice with oncologists and interdisciplinary rounding
- Experience with cost avoidance and drug therapy optimization
- Pharmacy residency precepting and staff development
