Roles & Responsibilities
- Provide skilled physical therapy evaluation and treatment for residents in a skilled nursing facility
- Develop individualized treatment plans that improve mobility, function, and quality of life
- Document resident progress clearly and timely, meeting facility and regulatory standards
- Collaborate with nursing and therapy colleagues to coordinate care and achieve patient goals
- Educate residents and families on safe mobility, fall prevention, and home exercise programs
- Participate in interdisciplinary team meetings and contribute to care planning
- Support quality improvement initiatives and maintain professional standards
Qualifications
- Licensed Physical Therapist in North Carolina or eligible for NC licensure
- DPT or equivalent PT degree (DPT listed as primary skill)
- Open to all experience levels — any experience accepted, including new grads
- Comfortable working in a skilled nursing facility (SNF) setting
- Strong communication skills and ability to work as part of a team
- Reliable, professional, and committed to resident-centered care
Education
- Doctor of Physical Therapy (DPT) preferred; PT degree required or equivalent
Knowledge & Skills
- Hands-on therapy skills focused on geriatric and post-acute populations
- Knowledge of SNF documentation and regulatory expectations
- Skilled in mobility, balance, gait training, and functional transfers
- Ability to assess functional status and set measurable goals
- Collaborative approach to care and strong interdisciplinary communication
