Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment for residents in a skilled nursing facility
• Develop individualized treatment plans that improve mobility, function, and quality of life
• Document resident progress clearly and timely, meeting facility and regulatory standards
• Collaborate with nursing and therapy colleagues to coordinate care and achieve patient goals
• Educate residents and families on safe mobility, fall prevention, and home exercise programs
• Participate in interdisciplinary team meetings and contribute to care planning
• Support quality improvement initiatives and maintain professional standards

Qualifications

• Licensed Physical Therapist in North Carolina or eligible for NC licensure
• DPT or equivalent PT degree (DPT listed as primary skill)
• Open to all experience levels — any experience accepted, including new grads
• Comfortable working in a skilled nursing facility (SNF) setting
• Strong communication skills and ability to work as part of a team
• Reliable, professional, and committed to resident-centered care

Education

• Doctor of Physical Therapy (DPT) preferred; PT degree required or equivalent

Knowledge & Skills

• Hands-on therapy skills focused on geriatric and post-acute populations
• Knowledge of SNF documentation and regulatory expectations
• Skilled in mobility, balance, gait training, and functional transfers
• Ability to assess functional status and set measurable goals
• Collaborative approach to care and strong interdisciplinary communication
