Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and follow the care plan safely and accurately
• Monitor and document vital signs, symptoms, and patient progress
• Collaborate with the care team to coordinate and improve patient outcomes
• Perform wound care, ADLs assistance, and other delegated nursing tasks
• Communicate clearly with families and the interdisciplinary team
• Maintain accurate, timely documentation in the patient record

Qualifications

• Active Licensed Practical Nurse (LPN) license
• Completion of an accredited practical nursing program
• Current BLS/CPR certification preferred or willing to obtain
• Comfortable working 12+ hour day shifts and adapting to clinical needs
• Strong communication skills and a patient-centered mindset

Education

• Graduate of an accredited Practical Nursing program (LPN)
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and accurate charting
• Basic wound care and infection control practices
• Patient assessment and vital sign monitoring
• Familiarity with electronic medical records and documentation
• Team collaboration, prioritization, and time management
