Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents according to plan of care
- Administer medications and treatments accurately and safely within LPN scope
- Monitor and document vital signs, patient status changes, and response to interventions
- Collaborate with RNs, physicians, and interdisciplinary team members to ensure continuity of care
- Perform wound care, basic assessments, and supportive nursing procedures
- Maintain clear, timely documentation in the medical record and communicate handoffs effectively
- Support patient mobility, activities of daily living, and comfort measures with dignity and respect
- Participate in infection control practices and promote a safe environment for residents and staff
Qualifications
- Current LPN licensure (Licensed Practical Nurse) — active and in good standing
- Basic Life Support (BLS/CPR) certification preferred or ability to obtain promptly
- Comfortable working night shift hours (6:00 PM - 6:30 AM) and able to follow per-diem scheduling
- Strong clinical assessment, medication administration, and documentation skills
- Excellent communication and teamwork skills; patient-focused and reliable
- Background check and any facility-required screenings completed prior to start
Education
- Graduate of an accredited practical nursing program (LPN diploma or equivalent)
Knowledge & Skills
- Proficient in medication administration and monitoring for side effects
- Skilled with vital signs, basic assessments, and recognition of patient changes
- Familiarity with long-term care/rehabilitation workflows and resident-centered care
- Competent documentation practices and ability to use electronic charting systems
- Infection control, wound care basics, and safe patient handling techniques
- Strong prioritization, time management, and teamwork abilities
