Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents according to plan of care
• Administer medications and treatments accurately and safely within LPN scope
• Monitor and document vital signs, patient status changes, and response to interventions
• Collaborate with RNs, physicians, and interdisciplinary team members to ensure continuity of care
• Perform wound care, basic assessments, and supportive nursing procedures
• Maintain clear, timely documentation in the medical record and communicate handoffs effectively
• Support patient mobility, activities of daily living, and comfort measures with dignity and respect
• Participate in infection control practices and promote a safe environment for residents and staff

Qualifications

• Current LPN licensure (Licensed Practical Nurse) — active and in good standing
• Basic Life Support (BLS/CPR) certification preferred or ability to obtain promptly
• Comfortable working night shift hours (6:00 PM - 6:30 AM) and able to follow per-diem scheduling
• Strong clinical assessment, medication administration, and documentation skills
• Excellent communication and teamwork skills; patient-focused and reliable
• Background check and any facility-required screenings completed prior to start

Education

• Graduate of an accredited practical nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Proficient in medication administration and monitoring for side effects
• Skilled with vital signs, basic assessments, and recognition of patient changes
• Familiarity with long-term care/rehabilitation workflows and resident-centered care
• Competent documentation practices and ability to use electronic charting systems
• Infection control, wound care basics, and safe patient handling techniques
• Strong prioritization, time management, and teamwork abilities
