Roles & Responsibilities
- Provide compassionate direct patient care within the LPN scope of practice
- Administer medications and treatments safely and accurately
- Monitor patient status and report changes promptly to the care team
- Document assessments, interventions, and patient responses in the medical record
- Collaborate with nurses, providers, and support staff to deliver coordinated care
- Educate patients and families about care plans, medications, and discharge instructions
- Promote infection control practices and maintain a safe environment
Qualifications
- Active Licensed Practical Nurse (LPN) license — New Hampshire licensure preferred or eligibility to obtain
- Current Basic Life Support (BLS) certification or willingness to obtain
- Previous LPN experience in a clinical or long-term care setting is preferred
- Up-to-date immunizations and ability to meet facility onboarding requirements
- Strong communication skills and a patient-centered attitude
Education
- LPN diploma or certificate from an accredited program
- High school diploma or GED required
Knowledge & Skills
- Competence in medication administration and basic clinical assessments
- Skilled in accurate and timely clinical documentation
- Knowledge of infection prevention and safety protocols
- Ability to work well in interdisciplinary teams and adapt to changing patient needs
- Comfortable using electronic medical records and standard clinical equipment
