Location: Lebanon, New Hampshire

Contract Type: Per-Diem

Shift Type: Per diem / variable days, 7:00 AM - 7:30 PM (Day shift)

Hours per Week: 36

Candidate Pay Rate: $33 per hour

Roles & Responsibilities

Provide compassionate direct patient care within the LPN scope of practice
Administer medications and treatments safely and accurately
Monitor patient status and report changes promptly to the care team
Document assessments, interventions, and patient responses in the medical record
Collaborate with nurses, providers, and support staff to deliver coordinated care
Educate patients and families about care plans, medications, and discharge instructions
Promote infection control practices and maintain a safe environment

Qualifications

Active Licensed Practical Nurse (LPN) license — New Hampshire licensure preferred or eligibility to obtain
Current Basic Life Support (BLS) certification or willingness to obtain
Previous LPN experience in a clinical or long-term care setting is preferred
Up-to-date immunizations and ability to meet facility onboarding requirements
Strong communication skills and a patient-centered attitude

Education

LPN diploma or certificate from an accredited program
High school diploma or GED required

Knowledge & Skills

Competence in medication administration and basic clinical assessments
Skilled in accurate and timely clinical documentation
Knowledge of infection prevention and safety protocols
Ability to work well in interdisciplinary teams and adapt to changing patient needs
Comfortable using electronic medical records and standard clinical equipment
