Roles & Responsibilities
- Provide safe, compassionate direct patient care following facility policies
- Administer medications and treatments as prescribed, documenting carefully
- Perform thorough assessments and report changes in patient condition
- Collaborate with the interdisciplinary team to create and update care plans
- Support patient and family education with clear, empathetic communication
- Maintain accurate, timely charting and handoffs to ensure continuity of care
- Adhere to infection control standards and promote a safe environment
Qualifications
- Current Licensed Practical Nurse (LPN) license in New Hampshire required
- Valid BLS/CPR certification required; ACLS a plus if applicable to area
- Comfortable administering medications and performing routine nursing procedures
- Previous clinical experience preferred but new grads from an LPN program are welcome to apply
- Strong communication skills and ability to work collaboratively in a team
Education
- Graduate of an accredited LPN/LVN program
- High school diploma or GED required
Knowledge & Skills
- Knowledge of basic nursing principles and medication administration
- Familiarity with patient assessment, wound care, and IV documentation where applicable
- Ability to prioritize workload in a fast-paced clinical environment
- Good charting skills and comfort with electronic medical record systems
- Strong interpersonal skills and a patient-centered approach
- Basic infection control and safety practices
