Roles & Responsibilities

• Provide compassionate, direct nursing care to patients in the assigned unit
• Administer medications and treatments accurately and document per policy
• Monitor patient condition and promptly report changes to the care team
• Perform routine assessments and record vital signs with attention to detail
• Collaborate with RNs, physicians, and support staff to deliver coordinated care
• Maintain accurate, timely documentation in the medical record
• Promote patient safety and infection control practices at all times
• Support patient and family education to encourage understanding and recovery

Qualifications

• Current, active Licensed Practical Nurse (LPN) credential — required
• BLS/CPR certification (or willingness to obtain promptly)
• Completion of an accredited LPN/LVN training program
• Ability to pass facility-required background and health screenings
• Comfortable working in a fast-paced clinical environment and collaborating with a care team
• Previous experience in long-term care or similar settings is a plus

Education

• Completion of an accredited LPN/LVN program
• High school diploma or GED

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic patient assessment and monitoring of vital signs
• Infection control and standard precautions
• Strong communication and bedside manner with patients and families
• Time management and ability to prioritize patient needs
• Familiarity with electronic medical records and charting
