Shift Type: In-home: Monday-Friday, 8:00 AM - 5:00 PM (day); In-patient: Rotating 12-hour shifts including days and nights (typical 7:00 AM - 7:00 PM and 7:00 PM - 7:00 AM), rotating schedule including weekends and on-call rotation
Hours per Week: 40
Candidate Pay Rate: $57 per hour

Roles & Responsibilities
- Provide skilled nursing care and case management for patients in their homes and at the inpatient hospice center
- Perform comprehensive patient assessments and create individualized care plans using the nursing process
- Coordinate and integrate services across disciplines to ensure continuity and high-quality outcomes
- Manage symptom control and provide patient and family teaching, counseling, and referrals
- Supervise and evaluate care provided by Home Health/Hospice Aides and CNAs to ensure plan of care compliance
- Maintain accurate, timely OASIS and patient documentation in accordance with agency standards and regulations
- Participate in team scheduling, on-call rotation, quality improvement activities, and in-service training
Qualifications
- Current Registered Nurse (RN) license in the State of Montana (required)
- Minimum 2 years recent experience in acute care, home care, or hospice (preferred)
- Valid driver’s license, reliable transportation, and appropriate auto insurance required
- Clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred
- Ability to participate in rotating 12-hour shifts and on-call schedules
- Strong communication and care coordination skills for interdisciplinary collaboration
Education
- Current RN license in the State of Montana (required)
- BSN preferred
Knowledge & Skills
- Proficiency in OASIS documentation and familiarity with home health/hospice documentation standards
- Skilled in patient assessment, care planning, and symptom management
- Experience with case management and coordinating community and support services
- Ability to teach and counsel patients and families to improve outcomes and quality of life
- Comfort participating in quality improvement activities and interdisciplinary team meetings
- Knowledge of current reimbursement rules and regulations as they relate to home health
