Roles & Responsibilities
- Evaluate patients and develop individualized treatment plans that promote recovery and mobility
- Provide skilled therapeutic interventions and progress care to meet functional goals
- Document patient progress clearly and timely to ensure continuity of care
- Collaborate with nurses, occupational therapists, physicians and family members for holistic care
- Educate patients and caregivers on home exercise programs and safety strategies
- Mentor and support PTA/assistive staff (when applicable) to maximize patient outcomes
Qualifications
- Licensed Physical Therapist (PT) or Physical Therapist Assistant (PTA) in good standing — NH licensure or ability to obtain
- Active BLS/CPR certification (or willingness to obtain prior to start)
- Strong clinical assessment and manual therapy skills relevant to rehabilitation
- Clear, compassionate communicator with excellent teamwork skills
- Previous inpatient or skilled rehab experience preferred
Education
- Physical Therapist: Doctor of Physical Therapy (DPT) or equivalent
- Physical Therapist Assistant: Associate degree from an accredited PTA program
Knowledge & Skills
- Skilled in patient assessment, treatment planning and progress measurement
- Proficient with therapeutic modalities and evidence-based rehab techniques
- Comfortable with documentation systems and regulatory compliance
- Ability to safely transfer and mobilize patients and train caregivers
- Strong time management and prioritization in a busy clinical setting
