Roles & Responsibilities
- Provide compassionate direct patient care to residents in a rehab and nursing setting
- Administer medications and treatments accurately and document clearly
- Monitor residents for changes in condition and communicate findings to the care team
- Support rehabilitation plans by reinforcing therapies and assisting with ADLs
- Collaborate with nurses, therapists, and leadership to deliver safe, high-quality care
- Maintain timely, professional documentation in the resident record
- Promote a safe, clean, and respectful environment for residents and families
Qualifications
- Current LPN license in Massachusetts required
- For RN applicants: active RN license in Massachusetts (facility hires both LPN and RN classes)
- Basic Life Support (BLS) certification required or must obtain upon hire
- Prior experience in long-term care, skilled nursing, or rehab preferred
- Strong communication skills and a team-oriented mindset
- Ability to work night shifts and adapt to per diem scheduling
Education
- Graduation from an accredited LPN program required; ADN/BSN for RN applicants
Knowledge & Skills
- Medication administration and documentation expertise
- Knowledge of wound care, ADLs, and rehab support
- Familiarity with electronic charting and care plan development
- Ability to assess resident condition and prioritize interventions
- Customer-focused approach to resident and family interactions
