Roles & Responsibilities
- Provide compassionate, high-quality nursing care to residents in a skilled nursing/rehab setting
- Administer medications and treatments safely and according to facility policy
- Monitor and document resident conditions, vital signs, and care interventions
- Collaborate with interdisciplinary team members to support individualized care plans
- Assist residents with activities of daily living and mobility to promote independence
- Report changes in condition promptly and participate in shift handoffs for continuity of care
- Maintain accurate and timely electronic and paper documentation in compliance with regulations
- Adhere to infection control practices and facility safety protocols to protect residents and staff
Qualifications
- Current, active LPN or RN license (able to practice as LPN or RN in Massachusetts preferred)
- Basic Life Support (BLS/CPR) certification required or must obtain upon hire
- Prior experience in long-term care, rehab, or skilled nursing is highly valued
- Strong communication skills and the ability to work well on an interdisciplinary team
- Current immunizations and ability to pass background and health screenings per facility policy
Education
- LPN: Vocational diploma or practical nursing certificate
- RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) for Registered Nurse applicants
Knowledge & Skills
- Solid clinical assessment skills and ability to recognize changes in resident condition
- Medication administration and documentation proficiency
- Familiarity with electronic medical records and accurate charting practices
- Knowledge of infection prevention, wound care basics, and safety protocols
- Time management, prioritization, and teamwork in a fast-paced care environment
- Compassionate resident-centered care and excellent interpersonal skills
