Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents during the night shift
- Administer medications and treatments accurately and on time
- Perform regular patient assessments and document changes in condition clearly
- Collaborate with the interdisciplinary team to support individualized care plans
- Monitor vitals, manage alarms, and respond promptly to urgent needs
- Support rehabilitation goals and assist with ADLs as needed
- Maintain accurate, timely electronic and paper charting while following facility protocols
Qualifications
- Active LPN license in the state of practice (LPN) — new grads encouraged to apply if licensed)
- Current Basic Life Support (BLS) certification or willingness to obtain upon hire
- Previous experience in long-term care, rehab, or skilled nursing preferred
- Strong communication skills and a patient-first attitude
- Ability to work independently on night shifts and escalate concerns appropriately
Education
- Graduate of an accredited practical nursing program (LPN) or equivalent credential
Knowledge & Skills
- Clinical assessment and monitoring of adult patients
- Medication administration and documentation best practices
- Familiarity with infection control and wound care principles
- Comfortable with electronic health records and timely charting
- Team collaboration and effective handoff communication
