Roles & Responsibilities

• Coordinate comprehensive pediatric case management services from admission through discharge to ensure smooth transitions of care
• Lead discharge planning and care coordination with families, multidisciplinary teams, and community resources
• Manage complex pediatric cases including NICU and PICU patients with compassion and clinical expertise
• Document thoroughly and accurately in Epic to support quality care and compliance
• Advocate for patients and families, aligning care plans with medical, social, and discharge needs
• Collaborate with physicians, social workers, therapists, and community partners to optimize outcomes
• Support throughput and capacity by assisting with patient flow and appropriate disposition planning

Qualifications

• Active CA RN License (required)
• AHA BLS certification (required)
• Minimum 2 years pediatric case management experience, including NICU or PICU experience (required)
• Experience with Epic and electronic charting systems (required)
• Travel assignment experience (required) and willingness to float to other UCSF facilities
• Experience with discharge planning, care coordination, and utilization review
• Comfort working with varying patient ratios and acuity, including experience with high ratios (noted 1:20+ experience)
• COVID-19 vaccine and booster required per facility guideline (medical or religious exemptions only)
• Master's in Nursing or healthcare related field (preferred)
• Community hospital or acute care experience (preferred)

Education

• Master's in Nursing or a healthcare-related field (preferred)

Knowledge & Skills

• Pediatric care across age groups: neonates, infants, adolescents
• Case management and utilization review best practices
• Discharge planning, resource identification, and care coordination
• Clinical familiarity with NICU and PICU populations
• Charting and documentation in Epic
• Interdisciplinary communication and team-based care
• Ability to manage complex cases and navigate community resources
