Roles & Responsibilities

• Coordinate patient care and lead discharge planning to ensure safe, timely transitions home or to the next level of care
• Perform utilization review and case management using Milliman Guidelines to support appropriate care decisions
• Develop and manage individualized plans of care, including needs assessments and ordering DME as required
• Document thoroughly and accurately in Epic to maintain continuity and compliance
• Collaborate with interdisciplinary teams — PT/OT, pharmacy, social services, respiratory, and more — to remove barriers to care
• Float within case management services as needed to cover gaps and support unit needs
• Provide patient and family education and act as an advocate for quality, efficient care

Qualifications

• Valid Alaska RN license in hand (required for auto-offer)
• BLS certification (required)
• ACM (Advanced Case Management) certification preferred
• Minimum 1 year acute care nursing experience (first-timers accepted)
• Epic EMR experience (required)
• Comfortable working day shifts and providing weekend coverage as scheduled
• Ability to float within scope to meet facility needs
• Candidates must have a permanent address at least 50 miles from the facility (locals not accepted)

Knowledge & Skills

• Proficiency with Epic charting and Epic hardstop workflows
• Knowledge of Milliman Guidelines and utilization review practices
• Strong care coordination, discharge planning, and needs assessment skills
• Experience ordering DME and developing actionable plans of care
• Familiarity with acute hospital workflows across populations (pediatrics, women’s services, ICU, med-surg, SDU/PCU/IMC/Obs)
• Comfortable using Microsoft Office and Microsoft Teams for communication and documentation
• Understanding of CMS and HIPAA regulatory requirements
