Shift Type: Monday-Friday, 7:30 AM - 4:00 PM (Day); every other weekend or 4 weekend days in a 4-week schedule
Hours per Week: 40
Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Coordinate patient care and lead discharge planning to ensure safe, timely transitions home or to the next level of care
- Perform utilization review and case management using Milliman Guidelines to support appropriate care decisions
- Develop and manage individualized plans of care, including needs assessments and ordering DME as required
- Document thoroughly and accurately in Epic to maintain continuity and compliance
- Collaborate with interdisciplinary teams — PT/OT, pharmacy, social services, respiratory, and more — to remove barriers to care
- Float within case management services as needed to cover gaps and support unit needs
- Provide patient and family education and act as an advocate for quality, efficient care
Qualifications
- Valid Alaska RN license in hand (required for auto-offer)
- BLS certification (required)
- ACM (Advanced Case Management) certification preferred
- Minimum 1 year acute care nursing experience (first-timers accepted)
- Epic EMR experience (required)
- Comfortable working day shifts and providing weekend coverage as scheduled
- Ability to float within scope to meet facility needs
- Candidates must have a permanent address at least 50 miles from the facility (locals not accepted)
Knowledge & Skills
- Proficiency with Epic charting and Epic hardstop workflows
- Knowledge of Milliman Guidelines and utilization review practices
- Strong care coordination, discharge planning, and needs assessment skills
- Experience ordering DME and developing actionable plans of care
- Familiarity with acute hospital workflows across populations (pediatrics, women’s services, ICU, med-surg, SDU/PCU/IMC/Obs)
- Comfortable using Microsoft Office and Microsoft Teams for communication and documentation
- Understanding of CMS and HIPAA regulatory requirements
