Shift Type: Monday-Friday, day shift. Typical start/end time options include 7:00 AM - 3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, or 8:30 AM - 5:00 PM. Weekend requirement: every other weekend (or 4 weekend days in a 4-week schedule). Day shift.

Roles & Responsibilities

• Coordinate comprehensive discharge planning and care transitions to ensure safe, timely patient discharges
• Perform utilization review and case management activities following Milliman Guidelines and CMS/HIPAA regulations
• Document thoroughly and accurately in Epic to keep the care team informed and the chart audit-ready
• Collaborate with interdisciplinary partners — social services, therapy teams, pharmacy, transportation and more — to remove barriers to care
• Conduct needs assessments and arrange DME and supportive services as part of individualized plan of care
• Provide timely care coordination across specialties (pediatrics through geriatrics) with a patient-centered approach
• Float within case management services to cover gaps and maintain consistent patient care coverage

Qualifications

• Active AK RN license in-hand required for auto-offer (screen for pending license; license in hand preferred)
• Minimum 1 year of acute care experience required; first-time travelers are accepted
• Epic EMR experience required (hardstop) — strong charting and documentation skills
• BLS certification required; ACM (Case Management) certification preferred
• Comfort working in a busy acute hospital (Magnet, Level II Trauma) and following CMS and HIPAA guidelines
• Ability to work every other weekend (or 4 weekend days in a 4-week schedule) and float within case management scope
• Candidates’ permanent address must be at least 50 miles from the facility (locals not accepted)

Knowledge & Skills

• Solid knowledge of case management, utilization review, discharge planning and care coordination
• Familiarity with Milliman Guidelines for utilization review
• Proficient use of Epic for charting and care documentation
• Experience ordering and coordinating DME and developing individualized plans of care
• Comfort collaborating with interdisciplinary teams including social work, PT/OT, pharmacy and rapid response
• Working familiarity with Microsoft Office and Teams (preferred)
• Understanding of CMS requirements and HIPAA regulations in an acute hospital setting
