Roles & Responsibilities
- Assess wounds and ostomies thoroughly and formulate individualized care plans that promote healing
- Perform wound treatments and dressing selection, including Wound Vac therapy, and advanced modalities
- Manage colostomy and ileostomy care, provide education and support to patients and caregivers
- Stage pressure ulcers accurately (Stages I–IV) and implement evidence-based interventions
- Perform and supervise appropriate debridement techniques and product use (alginates, foams, hydrocolloids, silver products)
- Document care and progress clearly in Epic to ensure continuity and quality of care
- Collaborate with the interdisciplinary team to coordinate rehabilitation-focused patient care
- Educate patients, families, and staff on continence devices, catheter management, and self-care techniques
Qualifications
- National Alliance of Wound Care and Ostomy Certification (WOCN) required
- BLS preferred
- Minimum 1 year of clinical experience (wound/ostomy focus preferred)
- Proficiency with Epic charting system (charting in Epic is required)
- Comfort floating between units as the position requires
- Ability to work block schedules and weekday-only shifts as needed
Knowledge & Skills
- Wound assessment and management (neuropathic, peripheral arterial, venous, pressure ulcers)
- Colostomy and ileostomy management and patient education
- Wound Vac therapy application and monitoring
- Pressure ulcer staging (Stage I–IV) and treatment planning
- Debridement methods: mechanical, autolytic, enzymatic, sharp
- Familiarity with wound dressings: alginates, foams, hydrocolloids, hydrogels, silver products
- Continence therapeutic devices and fecal/urinary incontinence programs
- Catheter management and teaching self-care techniques
- Strong documentation skills in Epic and interdisciplinary communication
