Roles & Responsibilities
- Provide skilled physical therapy assistance to long-term care residents in a SNF setting
- Assist with patient evaluations, treatment plans, mobility training, and therapeutic exercises
- Support safe transfers, gait training, and use of adaptive equipment to maximize independence
- Document patient progress accurately and timely in the facility EMR
- Collaborate with therapists, nursing, and interdisciplinary team members to coordinate care
- Educate patients and families on home programs and safe mobility strategies
- Maintain equipment, follow infection control protocols, and uphold facility policies
Qualifications
- Active California Physical Therapy Assistant (PTA) license or eligibility to obtain one
- Experience working in a Skilled Nursing Facility or long-term care environment preferred
- Basic Life Support (BLS) / CPR certification (or willingness to obtain)
- Comfortable completing full-day shifts within the 7 AM to 7 PM timeframe, Monday-Friday
- Strong documentation skills and ability to communicate clearly with patients and team
- Reliable, professional, and able to handle the physical demands of patient care
Education
- Graduate of an accredited Physical Therapy Assistant program (Associate degree or equivalent)
Knowledge & Skills
- Hands-on experience with therapeutic exercise, gait training, and transfers
- Knowledge of long-term care patient needs and SNF workflow
- Proficient with patient documentation and common EMR tasks
- Skilled in safe patient handling techniques and use of assistive devices
- Good clinical judgment, time management, and teamwork abilities
- Strong interpersonal skills for working with residents and families
